- Service Certificate of Parent (for Govt. employees)
- Transfer Details (if applicable)
- Medical Fitness Certificate (if required)
- Disability Certificate (for CwSN, if applicable)

- Birth Certificate (Issued by Competent Authority)
- Recent Passport Size Photograph
- Aadhar Card of Child
- Proof of Residence (Address Proof)
- Caste Certificate (if applicable)
- EWS/BPL Certificate (if applicable)
- Service Certificate (for Central Govt./PSU employees)
- Transfer Certificate / Service Transfer Details
- Certificate of Disability (if applicable)
- Single Girl Child Affidavit (if applicable)
